In accordance with
Connecticut General Statutes, 31-53 Certified Payrolls with
a statement of compliance shall be submitted monthly to the
contracting agency.
Note: Once you have downloaded these
forms and are ready to print them out, set the print function on your PC to the
horizontal print orientation.
Note2: Please download both the Payroll
Certification for Public Works Projects and the Certified Statement of
Compliance for a complete package. The Certified Statement of Compliance appears
on the same page as the Fringe Benefits Explanation page.
