AGENT STATE REQUEST FOR TRAINING APPROVAL

Per Trade Act regulations, the liable state is responsible for all determinations, redeterminations, and decisions on appeals on all claims for program benefits, including training.

If an agent state needs to submit a training plan to Connecticut (liable state) for review and approval, please use this link to download our Interstate Training Plan Checklist. This provides most of the necessary data needed to make a determination, however, additional information may be requested depending on individual circumstances.

We generally ask that the information be submitted at least two weeks prior to the start of training to give adequate time for review. After the form is completed, please use the contact information below to submit.

Once a decision is made, the agent state will be notified. If applicable, information will be sent to the participant with instructions on how to file for TRA.

Please email the completed form to each of the following:

Fran Stankiewicz - frances.stankiewicz@ct.gov
Chris Conte - christine.conte@ct.gov
Joe Criscuolo - joseph.criscuolo@ct.gov

The regulations pertaining to liable/agent states responsibilities can be found at:
§618.824 Liable State and agent State responsibilities
