A NOTE REGARDING ON-LINE FILLABLE
FORMS:
PDF to HTML Format Converter
Adobe provides
resources for visually
impaired users to facilitate the use of screen readers with
PDF documents. The tool below converts PDF documents on-the-fly to
an HTML format that is immediately returned to your browser.
If the converter is unable to provide satisfactory results, you may e-mail
dol.webhelp@ct.gov
to request a copy of the document. Please be specific regarding your request
by providing the name and/or number of the document or form.
Troubleshooting
If, after clicking on the PDF link and making
sure Adobe Acrobat Reader software is installed on your computer, you are
still experiencing difficulties opening a PDF document, try downloading the file
to your computer by doing the
following:
From Internet Explorer:
Repeat steps from above. Internet Explorer refers to saving
the document in step one, as "save target as".
