Workforce Investment Act Eligible Training Provider and Program WIA Application Forms and Instructions

In preparing and submitting ETPL documents, contact information including the address of the workforce investment board (WIB) area will be needed.  Please click here for contact information.

Before proceeding to WIA Application A or B, an original Pre-Application must be completed and submitted to the Lead WIB and a copy to the appropriate Secondary WIB(s).  Refer to the list and map included in the Pre-Application to determine the Lead WIB and, if applicable, Secondary WIB(s).  After receiving and reviewing the Pre-Application, the Lead WIB will direct the provider to complete Application A or B and submit it for review.
