SUBJECT: PY 2001 WIA MIS Data Collection Forms and Instructions and Training

Background: The second year of WIA implementation has begun.  While there is work beginning on a statewide case management computer system, during PY 2001, we will again be working with the IBM mainframe system we had previously modified to accommodate WIA data collection and reporting needs.

Over the first year of implementation, several issues developed involving the administration of the WIA programs and recording of data in the IBM system.  We recognized the need for more improvements in the WIA data collection system, and we worked to further modify the system so that it would better meet local area needs and still meet federal reporting requirements.

We feel the changes we have made to the system answer some of your concerns and requests for a system that is more responsive to the way the WIA programs are actually administered by the local areas.  At the same time, we have retained the ability to report out the required information to USDOL.

Policy: Effective July 1, 2001, please use the following forms and instructions for recording and entering relevant data on WIA program participants.  Please note that there have been some revisions to existing forms and we have developed some new forms.

The following are enclosed:  

  • Workforce Investment Act Registration for Title IB Services (WIA-1) (Rev. 7/01) and instructions.
  • WIA Activities Record (WIA-3) (Rev. 7/01) and instructions.
  • WIA Exit Record (WIA-2) (Rev. 7/01) and instructions.
  • WIA Placement Record (WIA-6) (N. 7/01) and instructions.
  • 1st Quarter After Exit Follow-Up (WIA-7) (N. 7/01) and instructions.
  • 3rd and 4th Quarter After Exit Follow-Up (WIA-8) (N. 7/01) and instructions.
  • WIA Goals Status Record (WIA-4) (N. 7/00) and instructions. (required for youth only)

Training will be held on August 2nd at the Central Office of the Department of Labor in the Staff Development Computer Room on the third floor.  Training will begin at 9:00 a.m.  Please be sure to confirm your attendance by July 27, 2001.  The room seats a limited number of individuals, so we ask that each area send one or two people at the most.
