SUBJECT: WIA MIS Data Collection Forms and Instructions

Background: As you are aware, we have been working for several months to "modify" the old JTPA IBM system for use as a WIA Title IB Data Collection system. That project has resulted in the development of the data collection forms and instructions contained in this issuance.

Policy: Effective July 1, 2000, the Workforce Investment Act replaces JTPA. The new WIA system required us to develop a number of data collection instruments and instructions included here. They are as follows:

  • Attachment I - WIA Registration and Instructions
  • Attachment II - WIA Activities Record and Instructions
  • Attachment III - WIA Exit/Placement Record and Instructions
  • Attachment IV - WIA Goals Status Record and Instructions
The new registration form should be used in the determination of eligibility for WIA Title I sources as of July 1, 2000.

The activities record (Attachment II), WIA Exit/Placement Record (Attachment III), and the Goal Status Record (Attachment IV) should be used for all activities for participants who were carried over into WIA from JTPA and new WIA participants as of July 1, 2000.
