The following are some general
guidelines for the Incumbent Worker Assessment Survey forms.
New survey forms have been designed.
You will use Form #52375
for the Employers and
Form #37018 for
Participants. No other survey forms will be accepted. Please dispose of
any other survey forms you may have.
The survey forms are available on
the WIOA Website for your convenience to download and reproduce. When duplicating, please make
sure that the copier glass is clean and free of dust or debris and is aligned
properly on the copier so that we get the best and most accurate results. Please do not fax any surveys.
When faxed, lines and other anomalies distort and compromise the survey
responses. Please continue to email or call
to request new Employer numbers so that the boxes requesting Employer Number on
the survey forms are accurate. Make sure you fill in your WIB Number
so that the results for your region will be calculated accurately.
Please note that due to loss of
staff in the Performance and Accountability Unit, we do not have personnel to
make corrections or redo forms with missing WIB or Employer Numbers.
