The Magnetic Payment Voucher (Form
UC-2MAG) is a payment voucher for those employers who file their quarterly
unemployment tax and wage information on magnetic media (FTP, Disk or
Cartridge). This form is not a tax return or a billing notice. You can
not use this form to report tax or wage information. If you have received this
form either you, or your payroll agent, has submitted magnetic media for past

past and want to file on the internet.
You had a payroll service that filed
and paid your tax and wage reports and you are now filing on your own behalf.
You have received this form and you no
longer have employees.
