My software doesn’t generate forms with a bar code. What should I do?

If you are using a tax software package that produces facsimiles of our forms and the generated forms do not contain the bar code, please contact your software company and advise them of this change. Many software companies have already submitted approved bar coded forms and updated packages may be available for the 2nd quarter 2006 filing. Below are the specifications necessary to create approved bar code forms:

Bar Code Specifications 

  1. Font: 3 of 9 (3/9)
  1. Data to be Bar coded: *Regnumber*  (Include asterisk in bar code before and after registration number; Registration number should not include hyphens and must be the entire registration number (all 7 digits, including leading zeros).
  1. Position on forms:

UC-2

     From the Left edge of form:  2.9"
     From the Top edge of form to the top of the bar code:  7.88"   
     From the Top edge form to the bottom of the bar code:  8.38" 

UC-5A

     From the Left edge: 5.5"
     From the Top edge to the top of the bar code: 1.7"
     From the Top edge to the bottom of the bar code: 2.2"
