Apprenticeship Self-Registration Packet for Approved Sponsors
This packet allows Connecticut employers who are already approved to sponsor an apprenticeship program to self-register their employees as apprentices. If you are not an approved sponsor and would like to become one, or would like to register a pre-apprentice (high school student) please contact the Office of Apprenticeship Training at (860) 263-6085.

PLEASE
NOTE THE FOLLOWING:
Forms 1 through 3 must be filled out completely and
correctly with appropriate fees enclosed to comply with the
self-registration option.
If the packet is incorrect it will be returned (with the enclosed fees) and the registration is void.
The apprentice registration date is the date the completed packet is received and approved by the Office of Apprenticeship Training at the Connecticut Department of Labor. The employer (sponsor) will be notified by e-mail or fax of the registration date. If you do not provide an e-mail address or fax number the registration date will be acknowledged by mail. When returning this packet, please provide a cover letter as to your preference for acknowledgement.
It is mandatory that you notify this office
via email
or letter regarding any terminations or completions of apprentices as
this can impact your ability to utilize the self-registration procedure
due to ratio requirements. In addition to the name of the apprentice,
notification of terminations must include the date of termination (day,
month, year) and cause.

Sections 1 and 2 of the agreement address general information and are self- explanatory.
Section 3 – Please enter the trade in which the
apprentice will be registered. It is important that the trade entered is
one for which your company is already an approved sponsor. You must refer
to your company’s individual standards for this information. For example,
do not list Electrician – E-2, if your company is only approved for
Electrician, Low Voltage – L-6. Fill in the total term, number of hours
and months (1000 hours = 6 months) in this section.

listing of apprenticeship work schedules here . If the employee to be registered
has previous registered on the job training hours that were
obtained in a registered apprenticeship program or on the job training
hours for being a Connecticut Vocational-Technical High School graduate or
an approved proprietary school graduate
Download approved list here and you wish to award credit, enter those hours in this section.
Also enter the high school (or GED) or Vocational Technical high school
that your employee graduated from, if applicable, and the year of
graduation. If you are requesting credit for your employee for being a
Connecticut Vocational-Technical High School graduate or an approved
proprietary school graduate you must enclose appropriate documentation
(proof of graduation such as a diploma, certificate) when you return the
completed packet. If your apprentice did not graduate this space can remain
blank.
Section 4 – If you are awarding previously registered credit to your employee please enter in the company where credit was obtained, dates employed and total hours of credit. If your employee does not have any previously registered credit, skip this section.
Section 5 - This is the wage progression chart that is part of the standards for your approved program. You must refer to your individual standards’ most current apprentice wage progression chart (form number AT-6) to complete this section. Transfer the wage information from the wage progression chart into this section of the agreement. Please note that previous credit may increase the hourly wage of your apprentice.

| Form 2: JOURNEYPERSON LIST |

Please enter all journeypersons (with license type
and number in occupationally licensed trades) that you employ on a full
time basis that are available to train your apprentices. Compare the ratio you currently
have with your projected ratio with this registration(s). Job site ratio
is 1 journeyperson directly supervising a maximum of one apprentice.
However you must comply with the minimum hiring requirements as stated on
the hiring ratio reference chart.

meet the minimum ratio requirements this registration cannot be accepted.
You must apply for ratio relief.

Connecticut General Statutes, Section 31-22r requires a registration fee of $60 per apprentice paid by the employer for those new apprentices registered throughout the year (July 1 through June 30) and a registration fee $50 paid by each employee being registered as an apprentice. Form 3 is the registration invoice for registering an apprentice. Please complete this invoice.
